  1. Plan offers excellent insurance benefits with health insurance premium rates as low as $79 per month.
  2. Medical, dental, vision, life, accidental death and dismemberment and critical illness coverage available.
  3. No medical tests are required for eligible employees to join the life or health plan at start-up.
  4. Employees under age 66 can join the plan.
  5. Medical coverage is available for your life.  It does not stop at age 65 or 70.
  6. You have range of health insurance options to choose from.
  7. Obtain excellent major medical maximum health insurance, up to $1 million.
  8. Plan offers a special spousal survivor benefit.
  9. There is full portability of your life and health coverage, even if you leave employment.
  10. Life and AD&D coverage up to $300,000 is available based on your annual salary and is offered without medical tests at start-up.
  11. Premium rates are as low as $26 per month for $100,000 of life and AD&D coverage.
  12. There is no restriction on assigning your life insurance cover to a bank, finance company or credit union.
  13. Your have an option to secure Critical Illness insurance coverage up to $150,000 and based on your annual salary.
  14. Premium rates are as low as $16.50 per month for $50,000 of critical illness coverage.
